Donation Wish List- Please consider helping the SNVC keep costs down!

Funds are available on a limited basis, for those in need of further subsidy. Please click for application and submit it to the clinic before making an appointment. Click here for Espanola  (Spanish). You may complete and drop off at the SNVC during business hours or you may complete the form at the SNVC.  Call 910 692-3499 (FIXX) to have the form mailed. Proof of qualification is required and must be submitted with the completed form.

Sources for financial aid are provided by several grant programs to the SNVC.  Service fees for those requesting financial assistance are substantially lower than the regular service fees charged by the SNVC.
